You are only being tested for the sexually transmitted diseases or infections/drug testing on the order form, and not for all possible sexually transmitted diseases, infections or other potentially abused substances.
If your test results are positive, we may ask you to supply a name and address to abide by your state's Department of Public Health legal requirements. By law, they cannot compromise your confidentiality.
While the tests are highly specific and highly sensitive, there is statistical possibility of a false-positive or false-negative result.
It is possible to have an "indeterminate" test result for a variety of reasons. Follow-up testing is recommended to provide confirmation.
You release and hold harmless the Company from any injury or personal damage that occurs while you are on Patient Service Center (PSC) premises.

Cancellations, order changes, or questions:
Once an order has been placed, we incur fees for processing and consultation, as well as fee for the doctor’s orders. Therefore, in most cases we cannot issue a full refund if you wish to cancel your order. To cancel your order and request a partial refund, you will need to call 1-866-926-4669.
By placing an order, you acknowledge that you have given your consent for the performance of an HIV blood test, and the release of the results as described above.
